"The good news is that you have less risk than the average consumer, and you have less risk than the average consumer, so it's not like you're going to be taking any over-the-counter drugs," says Dr. John Jenkins, a gastroenterologist at Yale University. "The risk is there's less likelihood of a heart attack, so you don't know what's going to happen."
Dr. Jenkins and Dr. David Blum of the New York-based Institute for Health and Clinical Excellence have found that people with heartburn do not need a prescription. While they don't realize that Nexium can be dangerous to people with a heart condition, they do know that it's not a miracle pill. The best treatment for heartburn is to use an over-the-counter drug like Nexium to prevent heartburn.
"The key to protecting your heart is that you're going to be taking it for the rest of your life," says Dr. Blum. "There's a lot of that happens in your life, and you're not going to have that long to live with a heart condition. It's going to come back and be like, 'Oh my God, I can't live this down now.'"
Mark B. Miller, a cardiologist at the Cleveland Clinic's Cleveland Clinic, offers the medication in three dosages, and he uses a patient-controlled infusion (Pali) system to take it every hour or so. The medication has a "safety and efficacy" window, he says, and it's recommended that patients know what the dose is.
"If you are going to take the medication and it's been for a long time, the safety and efficacy and the duration of the medication will be important," says Dr. Miller. "It's important to know the dosage to be accurate, so that you can get the best results from it."
It's also important to remember that Nexium can interact with other medications. Some people might be taking Nexium with a combination of blood thinners, which are used to lower blood pressure. This combination can lead to serious side effects, including heart problems like a bad case of the heart attack.
Blum and Dr. Miller are both interested in the long-term safety of the medication, and they are also involved in clinical trials. The results of these trials are published in the New England Journal of Medicine.
Blum is a gastroenterologist with the Cleveland Clinic's gastroenterology team and has worked in the field of gastroenterology since 1993. He is currently a professor of pathology and the urology and cardiology professor at the University of California, San Francisco.
